Abstract:

In dieser retrospektiven Studie wurden die Ergebnisse nach Arthroskopie eines anterolateralen Soft Tissue Impingement Syndroms des Sprunggelenks bei 27 Patienten mit chronischen Beschwerden nach Supinationstrauma untersucht. Bei allen Patienten sind konservative Therapieansätze erfolglos geblieben. Durchschnittlich 13 Monate (Min 2 – Max 144) nach dem Trauma unterzogen sich die Patienten der Arthroskopie. 21 Patienten hatten eine Synovitis, proliferiertes Synovialgewebe hatten 16 Patienten, in 23 Fällen lag eine pathologische Plica vor, 22mal war das ATFL verletzt. Oft wurden dies Pathologien von Knorpelläsionen (14), anterioren Osteophyten (10), Arthrosen (5) und Osteochondrosis dissecans (4) begleitet. Nach durchschnittlich 33 Monaten (min 7 – max 57) erfolgte eine klinische Nachuntersuchung sowie eine Fragebogenerhebung. Die Bewertung stützt sich auf die Parameter Schmerz, Bewegungsausmaß, Schwellung, Aktivitäten des täglichen Lebens, Instabilität, sportliche Aktivitäten, Gehhilfen, Kraft, Funktion und Patientenzufriedenheit. Die arthroskopische Intervention verringerte die Schmerzen (85%), verbesserte die Sportfähigkeit (84%), die Beweglichkeit (80%), das Stabilitätsgefühl (95%), die Kraft (88%) und die Funktionsfähigkeit (88%). Insgesamt zeigt sich bei den Patienten eine breite Zufriedenheit (78%) mit dem Ergebnis der Arthroskopie. In keinem Fall trat eine neurologische Schädigung auf. Patienten mit begleitenden Arthrosen, anterioren Osteophyten und Osteochondrosis dissecans behielten überdurchschnittlich oft Einschränkungen der Funktionsfähigkeit und der sportlichen Belastbarkeit. Bei 23 Patienten mit arthroskopisch bestätigtem anterolateralem Soft Tissue Impingement Syndrom konnten MRT Aufnahmen ausgewertet werden. Die Genauigkeit der MRT Aufnahmen lag bei proliferiertem Synovialgewebe im anterolateralen Raum bei 83%, bei Plica Syndrom bei 91%, bei Synovitis bei 70%, bei Läsionen des ATFL 78%, bei anterioren Osteophyten bei 100%, bei Knorpelläsionen bei 87%, bei Osteochondrosis dissecans bei 91%. Konventionelle MRT Aufnahmen des Sprunggelenks sind hilfreich bei der Diagnosestellung eines anterolateralen Impingement und um weitere Pathologien auszuschließen.

In a retrospective study we analyzed the results of arthroscopic treatment of anterolateral soft tissue impingement syndrom of the ankle in 27 patients who had chronic pain following inversion injury. All had failed to respond of conservative treatment. At an average of 13 months (min 2 – max 144) after injury patients underwent ankle arthroscopy, which showed synovitis in 21 patients, proliferative synovial thickening in 16 cases, pathological plicas in 23 cases, 22 torn anterior talofibular ligaments, often with associated chondromalacia (14), anterior osteophytes (10), arthrosis (5) and osteochondritis dissecans (4). At an average followup interval of 33 months (min 7 – max 57) patients were examined clinically, as well a questionnaire examination were conducted. The results were scored for the criteria pain, range of motion, swelling, activities of daily living, instability, athletic activity, walking aids, strength, function and patients´ subjective assessment. Arthroscopic intervention reduces pain (85%), improves athletic activity (84%), range of movement (80%), feeling of stability (95%), strength (88%) and ability of function (88%). Altogether the patients show a large subjective assessment (78%) with the results of the arthroscopy. In no cases we found iatrogenic neurologic damage. Patients with associated arthrosis, anterior osteophytes or osteochondrosis dissecans often have restrictions in function and athletic activities.
