Abstract:

In der Dissertation werden Auswirkungen sensomotorischer Inkongruenz beim Tastschreiben und der Bedienung von Pointing Devices (PC- Maus und Stift/Tablett) auf die muskuläre Koordination untersucht. Im Vordergrund steht dabei die nahezu zeitgleiche und damit möglicherweise dysfunktionale Erregung muskulärer Gegenspieler (Ko-Kontraktionen muskulärer Antagonisten). Dieser wird eine Mitwirkung bei der Entstehung chronifizierender muskuloskeletaler Schmerzstörungen im oberen Bewegungsapparat von Bildschirmarbeitskräften zugeschrieben. Die folgenden beiden Fragenstellungen wurden in zwei Experimenten untersucht: 1. Können bei hochgradig überlernten Bewegungen an Pointing Devices (PC-Maus und –Stift) muskuläre Ko-Kontraktionen durch eine Störung der Augen-Hand-Koordination (verzögerte visuelle Rückmeldung) ausgelöst werden und sind diese auch im passiven Organ beobachtbar (bilateraler Transfer)? 2. Ist die Entstehung muskulärer Ko-Kontraktionen bei ballistischen Fingerbewegungen (Tastschreiben) abhängig von thermischen Bedingungen und können letztere sich bei unilateraler Applizierung auch bilateral auf zentral-motorische Parameter wie z. B. die Tastendruckdauer auswirken? Nach Darstellung der arbeitswissenschaftlichen Bedeutung muskuloskeletaler Schmerzstörungen am PC-Arbeitsplatz werden psychomotorische Parameter der Bedienung von PC-Eingabegeräten beschrieben. Da der Rückmeldung aus peripheren Systemen wie Sehnen, Muskeln und Gelenken eine besondere Rolle beim Auftreten von Bewegungsschmerzen zukommen könnte, wird mittels bestehender Literatur (McCabe et al., 2005; Sorgatz, 2005; Blakemore et al., 2000; Fink et al., 1999; von Holst & Mittelstaedt, 1950) ein heuristisches Modell abgeleitet. Damit wird ein Zusammenhang von Empfindung und ausgeführter Bewegung mittels eines Vergleichsprozesses (Komparator) zwischen hinführender-rückgekoppelter (efferenter) und zurückführender (reafferenter) Information im Organismus postuliert. Treten beim Vergleich zwischen den Informationssträngen akut nicht behebbare Inkongruenzen auf, könnten sie zur Fehlererregung muskulärer Gegenspieler und damit über dysfunktionale Ko-Kontraktionen zum exzentrischen Kontrahieren im schwächeren Muskel führen; ein Prozess, der Faserrisse nach sich ziehen kann. Den daraus folgenden nozizeptiven Erregungen wird nach dem Pain Adaption Model (Lund et al., 1991) durch weitere Ko-Kontraktionen begegnet, die zu einer Hemmung des Bewegungsablaufs führen sollen. Solche Inkongruenzen zwischen Rückmeldungen über den intendierten und den tatsächlich erfolgenden Bewegungsablauf sollen in dieser Arbeit durch zwei Maßnahmen herbeigeführt werden. Zum einen durch eine Inkongruenz zwischen propriozeptiver Rückmeldung aus dem sich bewegenden Organ und der visuellen Kontrolle dieser Bewegung, zum anderen durch Veränderungen der thermischen Bedingung im Bewegungsorgan. In der ersten Studie wird die Wirkung visueller Verzögerung (und somit die Inkongruenz von visueller und propriozeptiver Rückmeldung) bei der Arbeit mit Pointing Devices (PC- Maus/ Stift-Tablett), in der zweiten Studie die Wirkung von nicht adäquaten zusätzlichen thermischen Reizen (Inkongruenz durch zusätzliche sensorische Information) bei unterschiedlicher lateraler Belastung während des 10-Finger-Tastschreibens untersucht. Die Ergebnisse der ersten Studie zeigen unter visueller Verzögerung erhöhte Ko-Kontraktionen, längere Reaktionszeiten sowie erhöhtes Beanspruchungsempfinden bei den Probandinnen. Dabei besteht unter visueller Verzögerung eine Abhängigkeit der muskulären Aktivität des bewegenden (ipsilateralen) sowie nicht bewegenden (kontralateralen) Arms von der Art des Eingabegeräts. Bei der PC-Maus ist die ipsilaterale Erhöhung muskulärer Ko-Kontraktionen auf die erhöhte Aktivität des Agonisten zurückführbar. Beim Stiftklick scheinen andere als die bei der Mausarbeit beteiligten agonistischen Muskeln aktiviert zu werden. Auffällig bei der Stiftarbeit ist eine verringerte ipsilaterale und erhöhte kontralaterale muskuläre Aktivierung unter visueller Verzögerung. Die Ergebnisse der zweiten Studie zeigen beim Schreiben an der Tastatur unter vorheriger Kältereizung eine erhöhte muskuläre Aktivität, höhere Ko-Kontraktionen und langsameres Tippen mit längerer Tastendruckdauer. Dabei ist die Veränderung der muskulären Aktivität aufgrund früherer thermischer Reizung unabhängig von dem sich bewegenden Organ (mit den Fingern der rechten oder linken Hand angeschlagenen Tasten). Wie bei der Arbeit mit der PC-Maus scheinen muskuläre Ko-Kontraktionen den Tastenanschlag erst zu ermöglichen. Die Veränderung der muskulären Aktivität ist je nach Art der Reizung abhängig vom Reizort, jedoch unabhängig von der sich bewegenden Seite. Beide Studien zeigen, dass Ko-Kontraktionen (im Verlauf der Fingerbewegungen) unter sensomotorischer Inkongruenz durch eine Aktivitätssteigerung des Flexors (Agonist) gekennzeichnet sind. Diese Ko-Kontraktionen unterschieden sich damit von denen, die bei Schmerzen zu beobachten sind (Aktivitätsabnahme des Agonisten bei Aktivitätszunahme des Antagonisten). Daraus resultierende Berechnungsgrundlagen eines Belastungs-Ko-Kontraktionsindex sowie das Einbeziehen von Schmerzpatienten in weitere Studien werden diskutiert.

The dissertation deals with the effect of sensorimotor incongruence on muscular co-contractions while working with pointing devices (pc-mouse or stylus) or touch typing on a pc-keyboard. The main focus covers the almost simultaneous and possibly dysfunctional activation of muscular antagonists. These co-contractions of muscular antagonists may lead to the appearance of chronic muscular pain disorders in the upper extremities of pc-workers. In two experiments, the following questions were investigated: 1.Is the disturbance of the eye-hand coordination through visual delay able to induce muscular co-contractions in automatic and highly over-learned movements (working with pointing devices) and are those co-contractions observable in the non-moving extremity (bilateral transfer)? 2. Is there a dependency between the appearance of muscular co-contractions in ballistic finger-movements and thermal stimulations and are the central mechanism, like keystroke duration, impaired during unilateral stimulations? After a summary of the relevant occupational science literature, the aspects of psychomotor functions using computer interfaces are described. Since the feedback of peripheral systems like tendons, muscles and wrists occupy an important role in the occurrence of movement pain, a heuristic model is developed through the help of literature (McCabe et al., 2005; Sorgatz, 2005; Blakemore et al., 2000; Fink et al., 1999; von Holst & Mittelstaedt, 1950). This model provides a description of the theoretical relationship between sensation and voluntary movement in the organism. A comparison process (comparator) between efferent and afferent information is postulated. If incongruence appears during the comparison process, a false activation of the antagonist may be occuring. Those dysfunctional co-contractions could lead to eccentric contractions in the weaker muscle, which may be followed by ruptures of muscle fibers. As a result of these, and according to the pain adaption model (Lund et al., 1991), nociceptive stimulation will lead to further co-contractions, which should inhibit the motion sequence. Incongruence between feedback of the intended and factual occurring movement should be realized by two arrangements. On the one hand, incongruence between proprioceptive feedback of a moving arm and the affiliated visual (control) information and, on the other hand, incongruence through changes of thermal conditions in the moving limb. In the first experiment, the effect of visual delay (in this case the effect of incongruence of visual and proprioceptive information) while working with pointing devices is investigated. In the second experiment, the effect of inappropriate additional thermal stimulation (incongruence through additional sensory information) in different lateral workload conditions is investigated while touch-typing on a pc-keyboard. The results of the first study reveal higher co-contraction, longer reaction times and higher perceived sense of demands in the visual delay condition of all subjects. There exists a dependency in the visual delay condition between muscular activity of the moving (ipsilateral) limb as well as the passive (contralateral) limb and the type of pointing device. The ipsilateral increase of muscular co-contractions is explained by the greater activity of the agonist while working with the pc-mouse. When clicking with the stylus, other agonist- muscles seem to be activated than when clicking with the mouse. A noticeable change of a decreased ipsilateral an increased contralateral muscular activity occurs in the in the visual delay condition while working with the stylus. The results of the second study show higher muscular activity, greater co-contractions and decreased typing pace with prolonged key-press duration while touch typing under previous cold stimulation. The change of muscular activity because of previous thermal stimulation is observable independently from the moving limb (keystrokes with the fingers of the right or left hand). It seems that, while working with the pc-mouse, muscular co-contractions seem to make the keystroke possible. The change of muscular activity varies as a function of thermal input with the site of stimulation but independently from the moving limb. Both experiments show that co-contractions (within the lapse of a keystroke) are characterized by increasing activity in the flexor muscle (agonist) in the incongruence conditions. These co-contractions differ from those which are observable in pain conditions (decreased activity of the agonist muscle and increased activity of the antagonist muscle). The resulting calculations for a “muscular-strain” co-contraction index and further studies involving RSI-patients will be discussed.
